Position:SPIRITUAL COUNSELOR PRN

Type:PRN

Description

Reporting to the Manager of Spiritual Care Services, provides spiritual care support services to patients and families of Coastal Hospice at peak times of patient census, and assist with coverage during absences of assigned Spiritual Counselors.

Job Responsibilities:

  • Appropriately assesses spiritual care needs, which requires an understanding of definitions and principles of spiritual beliefs and practices, and a commitment to supporting people in addressing issues of spiritual suffering and distress at end of life.
  • In collaboration with the Interdisciplinary Group, develops and updates spiritual care plans for assigned patients and families.
  • Evaluates the effectiveness of interventions, adapting the plan of care accordingly.
  • Utilizes community resources to enhance spiritual care services.

Requirements

  • Experience and education requirements: MDiv, MTh, or Master’s in Pastoral Counseling and a minimum of 1 unit of clinical pastoral education.
  • A compassionate and collaborative spirit, and respect for diverse spiritual beliefs and practices.
  • Commitment to maintaining confidential patient information within an electronic medical record system.
  • Flexibility of scheduling. Assistance with some weekend coverage as needed.
  • Travel to patient’s homes as required.

Position:CERTIFIED MEDICAL TECHNICIAN - DAY

Type:Full Time

Description

Provides personal care services and administers medication to the terminally ill patient and performs related tasks at Stansell House, in accordance with the plan of treatment as assigned by the case manager. Behaves in a manner that consistently conveys commitment to the job of providing around-the-clock, compassionate and professional care.

Job Responsibilities:

DIRECT PATIENT CARE

  • Assists in feeding patients (if necessary) and provides nourishment between meals.
  • Answers call bells promptly to respond to patient/family questions or concerns.
  • Performs blood sugar finger sticks and documents results.
  • Takes and records patient vital signs as directed by RN.
  • Assists patients to bathroom or in using bedpan, urinal or bedside commode.
  • Gives personal care including baths, back rubs, oral hygiene, shampoos and changes bed linen as often as necessary.
  • Makes rounds per SH policy.
  • Collaborates as a team member in order to meet patients’ plan of care.
  • Administers medications within scope of practice per certification.
  • Responsible for daily cleaning of patient rooms.
  • Participates in weekend/holiday rotation to meet patient needs.

ASSISTS WITH UNIT OPERATIONS BY:

  • Washing of patients’ laundry, ensuring a clean supply of bed linens, patient clothing, etc.
  • Light meal preparation.
  • Maintaining safe physical environment according to unit needs and regulatory standards.

Requirements:

MINIMUM QUALIFICATIONS

  • Possesses a high school degree or general equivalency degree and is at least eighteen years of age.
  • Possesses a current certification to practice as a Certified Nursing Assistant with the Board of Nursing in Maryland (MBON).
  • Possess valid/current Certified Medication Technician certification with MBON.
  • Possess valid/current Certified Nursing Assist or GNA license.
  • Current CPR certification for Healthcare Provider (BLS).

Position:EXTENDED SERVICE NURSE - EVENING/WEEKEND PRN

Evening and Weekend Shifts – PRN

 
Functions:
 
  • Provides a consistent high quality hospice experience for every patient and family served by:
          a.     Triaging phone calls to identify patient and family needs
          b.     Monitoring and coordinating with extended services staff to complete home and hospital visits as appropriate
  • Utilizes the nursing process and coordinates with other IDT members to provide for the physical, spiritual, psychosocial, financial and bereavement needs of the patient and family.
  • Practices safe medication management according to professional standards which requires successful completion of medication mathematic skills competencies.
  • Proactively communicates with appropriate Manager/On Call/designee to ensure patient needs are met in a timely manner
  • Coordinates with consultant pharmacist and attending physician to optimize symptom management.
  • Maintains continuity and consistency of care by:
          a.     Recording all pertinent information in a concise, effective manner according to organizational standards.
          b.     Completing documentation in the patient’s electronic medical record by the end of the assigned shift.
          c.     Providing hand off communication according to established policy and procedure.

Job Requirements:

  • Current license to practice in Maryland as a Registered Nurse
  • Current CPR certification for Healthcare Provider (BLS )
  • The ability to lift, push, pull or move patients, equipment, supplies, etc. in excess of 50 pounds with or without the use of  adaptive/assistive devices.

2. Ability to work independently, efficiently and to make sound nursing judgments utilizing critical thinking skills with a minimum of direct supervision.

3. Superior physical assessment skills.

4. Current CPR certification for Healthcare Provider (BLS).

5. Sight, hearing and manual dexterity to function with or without prosthetics use that will enable these senses to function adequately to fulfill the requirements of the job.

6. The ability to lift, push, pull or move patients, equipment, supplies, etc. in excess of 50 pounds with or without the use of adaptive/assistive devices.

7. The ability to negotiate level and unlevel surfaces with or without the use of assistive devices.

8. Access to a vehicle.

Position:THRIFT SHOP ASSISTANT - PART TIME

Type:Part Time

Description

*Responsible for customer service functions including the accurate operation

of the sales cash register

*Insures the end of day cash/credit reconciliation is accurate and recorded on summary sheet

*Prices and displays items for sale in the register area

*Assists in stocking and displays as needed

*Other related duties as assigned by Store Manager

– Two 12 hour shifts (Friday and Saturday) 8:00pm – 8:00am

Position:MEDICAL SOCIAL WORKER

Type:Full Time

Seeking a Master’s prepared Medical Social Worker to join our compassionate interdisciplinary team providing a premier hospice experience for our patients and families. Manages a case load which requires travel to patients’ residences throughout our service area which includes Wicomico, Dorchester, Somerset and Worcester counties on Maryland’s lower Eastern Shore. Works collaboratively with other social workers to meet program needs. May assist in supervision/mentoring of social work interns, as needed.  LMSW, LCSW or LCSW-C required.

Position:HOSPICE AIDES

CNA or GNA. Requires at least one year previous experience in home care or geriatric setting.
