Position:CERTIFIED MEDICATION TECHNICIAN - DAY

Type:Full Time

Description

Provides personal care services and administers medication to the terminally ill patient and performs related tasks at Stansell House, in accordance with the plan of treatment as assigned by the case manager. Behaves in a manner that consistently conveys commitment to the job of providing around-the-clock, compassionate and professional care.

Responsibilities & Functions

DIRECT PATIENT CARE

  • Assists in feeding patients (if necessary) and provides nourishment between meals.
  • Answers call bells promptly to respond to patient/family questions or concerns.
  • Takes and records patient vital signs as directed by RN.
  • Assists patients to bathroom or in using bedpan, urinal or bedside commode.
  • Gives personal care including baths, back rubs, oral hygiene, shampoos and changes bed linen as often as necessary.
  • Collaborates as a team member in order to meet patients’ plan of care.
  • Administers medications within scope of practice per certification.
  • Responsible for daily cleaning of patient rooms.
  • Participates in weekend/holiday rotation to meet patient needs.

ASSISTS WITH UNIT OPERATIONS BY:

  • Washing of patients’ laundry, ensuring a clean supply of bed linens, patient clothing, etc.
  • Light meal preparation.

Requirements

  • Possesses a high school degree or general equivalency degree and is at least eighteen years of age.
  • Possesses a current certification to practice as a Certified Nursing Assistant with the Board of Nursing in Maryland (MBON).
  • Possess valid/current Certified Medication Technician certification with MBON.
  • Possess valid/current Certified Nursing Assist or GNA license.
  • Current CPR certification for Healthcare Provider (BLS).

Position:HOSPICE AIDE - DAY

Type:Full Time

Responsibilities & Functions
  • Provides direct patient care as instructed by a RN and/or physical therapist
  • Provides emotional support to the patient and family which requires effective human relation skills
  • Communicates effectively and timely with supervising nurse and other hospice aides by performing change of shift rounds

Requirements

  • CNA or GNA
  • At least one year previous experience in home care or geriatric setting

Position:NURSE PRACTITIONER

Type:Full Time

Shift: 8:00 AM – 5:00 PM (Monday through Friday)

Responsibilities & Functions

  • Performs consults as needed in the inpatient and outpatient palliative care settings under arrangement with Coastal Hospice, Inc.
  • Assesses patient’s physical, emotional and spiritual needs and documents in the electronic medical record.
  • Prescribes appropriate diagnostic and/or therapeutic measures to achieve patient/family goals and documents in the electronic medical record.
  • Facilitates family meetings for goals of care and discussions of advanced directives.
  • Provides face-to-face visits to hospice patients to assist in determining hospice eligibility. This requires driving to individual patient homes throughout our 4-county service area.
  • Makes rounds on hospice general inpatient unit for the purpose of symptom management and education.
  • Promotes and provides education related to palliative care and end of life.
  • Participates in the development of data collection tools, communication processes and other details of the consult service implementation and evaluation.
  • Collaborates across care settings to facilitate optimal coordination of patient-centered care.
  • May be required to travel in Coastal Hospice’s regional service areas.
Requirements
  • Current Maryland licensure as a Nurse Practitioner in adult or family practice.
  • Two years experience in related field preferred.
  • CPR Certification.
  • Proficiency in pain and symptom management.

Position:LPN

Type:Full Time or Part Time

Night Shift: 8:00 PM – 8:00 AM

Responsibilities & Functions

  • Provides skilled nursing to patients in accordance with physician orders and individual plan of care including medication administration, wound care, personal care tasks, and other prescribed treatments.
  • Participates in weekend/holiday schedule rotation.
  • Provides emotional support to patients/families.
  • Gives personal patient care including baths, back rubs, oral hygiene, shampoo, clothing and bedding changes. Uses patient assistance devices such as Hoyer lift, transfer boards, and draw sheets.
  • Documents services at time of care, assures documentation is included in patients’ medical records by the end of current shift.
  • Collects information on patient’s needs including physical, spiritual, and psychosocial matters and documents those in the electronic medical record and includes in shift report.
  • Obtains orders from physician as required and documents that information into patients’ medical record and includes the information in shift report.
  • Confers regularly with RN Care Coordinator and Stansell House Team Leader on patient and family status.
  • Coordinates activities of the C N A staff.
  • May perform additional duties as required such as meal preparation, laundry, or housekeeping as needed for continued operational continuity.

Requirements

  • Licensed Practical Nurse with current license from the Maryland Board of Nursing or compact state as appropriate.
  • Proficiency in patient care skills and symptom management.
  • Two year’s clinical experience (prefer Hospice)

Position:HOMEMAKER

Type:Part Time

Responsibilities & Functions

  • Prepares and delivers breakfast and lunch, and pre-prepares the evening meal for completion by other Stansell House staff
  • Monitors hospitality inventory and reports in writing to Team Leader
  • Restocks pantry shelves as needed
  • Assures kitchen is neat and clean
  • Maintains the cleanliness of the common areas including: sun room, family areas, spa, and outside porch and patio
  • Documents productivity in a daily time log in electronic record

Requirements

  • Work schedule is weekly, Saturday and Sunday, part time, flexible hours
  • High school diploma or equivalent
  • Previous experience in hospitality industry preferred
  • Must be familiar with food preparation skills and service

Position:THRIFT SHOP ASSOCIATE - PART TIME

Type:Part Time

Responsibilities & Functions

  • Prepares merchandise for sale and display; including sorting, washing, hanging and moving to sales floor as appropriate.
  • Prepares houseware items by hand cleaning or running through dishwasher.
  • Assists in removing seasonal items from sales floor and follows Thrift Shop SOP for storing and recycling merchandise, seasonal items.
  • Prices merchandise.
  • Displays merchandise in an appealing way.
  • May open and close store as needed.
  • May operate cash register as needed and compute end of day total cash register reports.
Requirements
  • Customer service attitude; works well with others, relates positively and courteously with a variety of people and co-workers.
  • Creates a positive image of Coastal Hospice in all customer transactions.
  • Ability to lift up to 50 pounds without assistance 80% of the shift and stand for up to four hours.
  • Ability to maintain accurate records.
