Position:EXTENDED SERVICE NURSE - EVENING/WEEKEND PRN

Evening and Weekend Shifts – PRN

 
Functions:
 
  • Provides a consistent high quality hospice experience for every patient and family served by:
          a.     Triaging phone calls to identify patient and family needs
          b.     Monitoring and coordinating with extended services staff to complete home and hospital visits as appropriate
  • Utilizes the nursing process and coordinates with other IDT members to provide for the physical, spiritual, psychosocial, financial and bereavement needs of the patient and family.
  • Practices safe medication management according to professional standards which requires successful completion of medication mathematic skills competencies.
  • Proactively communicates with appropriate Manager/On Call/designee to ensure patient needs are met in a timely manner
  • Coordinates with consultant pharmacist and attending physician to optimize symptom management.
  • Maintains continuity and consistency of care by:
          a.     Recording all pertinent information in a concise, effective manner according to organizational standards.
          b.     Completing documentation in the patient’s electronic medical record by the end of the assigned shift.
          c.     Providing hand off communication according to established policy and procedure.

Position:VICE PRESIDENT OF FINANCE

Description

Reporting to the President, responsible for the financial control and accounting operations, computer network administration of Coastal Hospice

Responsibilities and Functions

  • Establishes and maintains an accounting and financial system of impeccable quality and compliance
  • Collaborates with CEO to insure financial control of operations, capital and operating budgets, cash management, and reporting/compliance
  • Operational expertise using financial, patient accounting systems, Medicare and applicable government agencies reporting requirements
  • Provides collaborative and accurate internal budget creation, monitoring, reporting, and communications
  • Insures all internal financial statements and external reports to business partners are on time and accurate
  • Responsible for risk management of Coastal Hospice’s assets
  • Insures all compliance required by Federal, State, and County regulations and requirements
  • Directs operations and assures employee engagement and training of accounting staff
  • Business and thought partner to the CEO/President and Senior Leadership Team

Requirements

  • Demonstrates excellent customer service skills for external and internal partners
  • Bachelors Degree in Accounting/Finance

5. Sight, hearing and manual dexterity to function with or without prosthetics use that will enable these senses to function adequately to fulfill the requirements of the job.

7. The ability to negotiate level and unlevel surfaces with or without the use of assistive devices.

Position:CERTIFIED MEDICAL TECHNICIAN - NIGHT

Type:Full Time or Part Time

Description

Provides personal care services and administers medication to the terminally ill patient and performs related tasks at Stansell House, in accordance with the plan of treatment as assigned by the case manager. Behaves in a manner that consistently conveys commitment to the job of providing around-the-clock, compassionate and professional care

Job Responsibilities:

DIRECT PATIENT CARE

  • Assists in feeding patients (if necessary) and provides nourishment between meals.
  • Answers call bells promptly to respond to patient/family questions or concerns.
  • Performs blood sugar finger sticks and documents results.
  • Takes and records patient vital signs as directed by RN.
  • Assists patients to bathroom or in using bedpan, urinal or bedside commode.
  • Gives personal care including baths, back rubs, oral hygiene, shampoos and changes bed linen as often as necessary.
  • Makes rounds per SH policy.
  • Collaborates as a team member in order to meet patients’ plan of care.
  • Administers medications within scope of practice per certification.
  • Responsible for daily cleaning of patient rooms.
  • Participates in weekend/holiday rotation to meet patient needs.

ASSISTS WITH UNIT OPERATIONS BY:

  • Washing of patients’ laundry, ensuring a clean supply of bed linens, patient clothing, etc.
  • Light meal preparation.
  • Maintaining safe physical environment according to unit needs and regulatory standards.

Requirements:

MINIMUM QUALIFICATIONS

  • Possesses a high school degree or general equivalency degree and is at least eighteen years of age.
  • Possesses a current certification to practice as a Certified Nursing Assistant with the Board of Nursing in Maryland (MBON).
  • Possess valid/current Certified Medication Technician certification with MBON.
  • Possess valid/current Certified Nursing Assist or GNA license.
  • Current CPR certification for Healthcare Provider (BLS).

Position:ACCESS COORDINATOR

Type:Full Time

Job Responsibilities

  • Initiates referral process based on agency guidelines. Responds promptly to inquiries about hospice and Compass services
  • Ensures accurate referral and admission information in EMR
  • Ensures continuity and quality of care by expediting the transition from referral to admission
  • Collects relevant patient information from a variety of healthcare sources to add to clinical documentation
  • Obtains physician/nurse practitioner orders and coordinates supportive needs including pharmacy and DME
  • Follows up on referrals previously not taken under care
  •  Actively participates in professional development

Requirements

  • Excellent customer service skills with both internal and external customers
  • Current license to practice in the State of Maryland as a Licensed Practical Nurse
  • Ability to be credentialed as a Coastal Hospice representative in the region’s hospitals
  • Solid working knowledge of hospice usually associated with at least 2 years of LPN experience with previous hospice experience, highly desirable
  • Working knowledge of basic computer programs, such as Word, Excel, and Outlook mail. Ability to use electronic medical record system(s), fax machines, scanning, and copiers
  • Access to a personal vehicle in order to be able to travel to any facility or residence in the region and good driving record is required

– 24 hours per week, paid as 40 hours

– Two 12 hour shifts (Friday and Saturday) 8:00pm – 8:00am

Position:MEDICAL SOCIAL WORKER - PRN

Description

Provides social work services to patients and families in collaboration with other IDG members, volunteers and community resources.

Responsibilities and Functions:

  •  Provides a consistent high quality hospice experience for every patient and family served by:

a. Soliciting and identifying patient and family goals

b. Complying with established social work visit structure

c. Flexibility to manage frequency and duration of visits based on patient/family needs

  • Provides assessment, planning, intervention and ongoing evaluation of psychosocial function to meet assigned patient and family needs/goals by:

a. Making recommendations to members of the IDG on ways to support the patient and/or family.

c. Actively participating in IDG and family meetings to provide input on patient care and team support.

d. Providing patient/family counseling related to planned health care services.

e. Utilizing community resources to assist the patient/family to meet goals.

  • Develops and provides education to Coastal Hospice staff, volunteers and community members regarding the emotional and social dimension of hospice patients.
  • Maintains continuity and consistency of care by:

a. Recording all pertinent information in a concise, effective manner according to organizational standards.

b. Completing documentation in the patient’s electronic medical record.

c. Providing hand off communication according to established policy and procedure.

  • Promotes high quality patient care and team operations by:

a. Supervises social work interns as requested by the Team Leader and/or Social Work Resource Coordinator.

b. Maintaining professional relationships with referral sources, community agencies and healthcare partners.

c. Participating in the on-call rotation for weekend coverage as assigned.

Requirements

  • Masters degree in Social Work from an accredited school.
  • 2 years experience in hospital, long term care or hospice setting.
  • Current license in the state of Maryland, LCSW preferred.

Position:EXECUTIVE/HR ASSISTANT

Description

Provides confidential and accurate day to day functions, communications, and support to the Human Resources department and the office of the President/Chief Executive Officer. Reports to the Vice President of Human Resources and the President/Chief Executive Officer of the organization.

Job Responsibilities and Functions

  • Provides excellent administrative support for the President/CEO and the Board

Position:MEDICAL SOCIAL WORKER

Seeking a Master’s prepared Medical Social Worker to join our compassionate interdisciplinary team providing a premier hospice experience for our patients and families. Manages a case load which requires travel to patients’ residences throughout our service area which includes Wicomico, Dorchester, Somerset and Worcester counties on Maryland’s lower Eastern Shore. Works collaboratively with other social workers to meet program needs. May assist in supervision/mentoring of social work interns, as needed.  LMSW, LCSW or LCSW-C required.
