Position:CERTIFIED MEDICATION TECHNICIAN - DAY

Type:Full Time

Description

Provides personal care services and administers medication to the terminally ill patient and performs related tasks at Stansell House, in accordance with the plan of treatment as assigned by the case manager. Behaves in a manner that consistently conveys commitment to the job of providing around-the-clock, compassionate and professional care.

Responsibilities & Functions

DIRECT PATIENT CARE

  • Assists in feeding patients (if necessary) and provides nourishment between meals.
  • Answers call bells promptly to respond to patient/family questions or concerns.
  • Takes and records patient vital signs as directed by RN.
  • Assists patients to bathroom or in using bedpan, urinal or bedside commode.
  • Gives personal care including baths, back rubs, oral hygiene, shampoos and changes bed linen as often as necessary.
  • Collaborates as a team member in order to meet patients’ plan of care.
  • Administers medications within scope of practice per certification.
  • Responsible for daily cleaning of patient rooms.
  • Participates in weekend/holiday rotation to meet patient needs.

ASSISTS WITH UNIT OPERATIONS BY:

  • Washing of patients’ laundry, ensuring a clean supply of bed linens, patient clothing, etc.
  • Light meal preparation.

Requirements

  • Possesses a high school degree or general equivalency degree and is at least eighteen years of age.
  • Possesses a current certification to practice as a Certified Nursing Assistant with the Board of Nursing in Maryland (MBON).
  • Possess valid/current Certified Medication Technician certification with MBON.
  • Possess valid/current Certified Nursing Assist or GNA license.
  • Current CPR certification for Healthcare Provider (BLS).

Position:EXTENDED SERVICE NURSE - EVENING/WEEKEND PRN

Evening and Weekend Shifts – PRN

 
Responsibilities & Functions
 
  • Provides a consistent high quality hospice experience for every patient and family served by:
               – Triaging phone calls to identify patient and family needs
               – Monitoring and coordinating with extended services staff to complete home and hospital visits as appropriate
  • Utilizes the nursing process and coordinates with other IDT members to provide for the physical, spiritual, psychosocial, financial and bereavement needs of the patient and family.
  • Practices safe medication management according to professional standards which requires successful completion of medication mathematic skills competencies.
  • Proactively communicates with appropriate Manager/On Call/designee to ensure patient needs are met in a timely manner
  • Coordinates with consultant pharmacist and attending physician to optimize symptom management.
  • Maintains continuity and consistency of care by:
             –  Recording all pertinent information in a concise, effective manner according to organizational standards.
             – Completing documentation in the patient’s electronic medical record by the end of the assigned shift.
              – Providing hand off communication according to established policy and procedure.

Requirements

  • Current license to practice in Maryland as a Registered Nurse
  • Current CPR certification for Healthcare Provider (BLS )
  • The ability to lift, push, pull or move patients, equipment, supplies, etc. in excess of 50 pounds with or without the use of  adaptive/assistive devices.

Position:NURSE, EXTENDED SERVICES - PRN WEEKEND

Type:Part Time

PRN – Every Other Weekend

Position:HOSPICE AIDE - DAY

Responsibilities & Functions
  • Provides direct patient care as instructed by a RN and/or physical therapist
  • Provides emotional support to the patient and family which requires effective human relation skills
  • Communicates effectively and timely with supervising nurse and other hospice aides by performing change of shift rounds

Requirements

  • CNA or GNA
  • At least one year previous experience in home care or geriatric setting.

Position:PAYROLL ASSISTANT

Type:Part Time

Part Time position working twenty (20) hours per week on a set schedule.

Reports to the Payroll Coordinator and will assist with the payroll processing and serve as backup for the Payroll Coordinator.

Responsibilities and Functions

  • Processes employee time reports and verifies accuracy for payroll input
  • Prepares and reconciles Payroll System Input Form to Staff Services Reports
  • Enters and transmits payroll for processing to payroll processing vendor
  • Reviews completed payroll processors reports and verifies YTD totals
  • Works with payroll processing vendor to maintain database and add/delete department codes, etc. as needed
  • Verifies and ensures accuracy of Quarterly and Annual Payroll reports
  • Downloads monthly general ledger interface report from payroll application and prepares monthly payroll entries
  • Ensures payroll deductions are properly authorized, recorded, and corresponding payments are submitted for payment to vendors in a timely manner
  • Maintains files of employee Staff Service Reports
  • Assists Human Resources Vice President with special projects as assigned
  • Demonstrates excellent customer service for both internal and external customers

Requirements

  • Bachelor’s or Associate’s degree in Accounting or Business Administration or relevant work experience
  • Accuracy and attention to detail
  • Superior organizational ability
  • Excellent speaking and writing skills
  • Knowledge of and experience with computerized accounting programs
  • Working knowledge of personal computer applications for spreadsheets, word processing and data base programs
  • Excellent communications skills and ability to maintain effective and cordial relationships with hospice associates, including patients/families, vendors, physician’s offices and insurance companies
  • Demonstrated knowledge of accounting theory and accepted practice

Position:PALLIATIVE CARE NURSE PRACTITIONER

Description

Responsible for coordinating palliative care and consult service referrals at contracted hospitals; collaborates with hospice medical director and physicians, hospital and hospice management and staff, and palliative consult team members; receives clinical supervision from hospice Medical Director.

Responsibilities and Functions

  • Functions under the direction and supervision of the collaborating physician in the assessment, diagnosis, treatment, and evaluation of the patient with acute and/or chronic illness
  • Care of patients and families in critical care areas by the Palliative Care Nurse Practitioner will be limited to the palliative scope of practice; assessment and treatment planning in symptom management and end of life care
  • Obtains and records a problem-oriented health history including chief complaint, history of present illness, past history, family history, social history, and review of systems
  • Performs and records a complete physical exam with assessment of normal and abnormal findings
  • Performs patient and family teaching in prevention, promotion, and health maintenance especially related to symptom management and end of life care. Serves as patient advocate.
  • Collaborates with Care Coordinators, Hospital Clinical Director, and Palliative Team in assisting the patient and/or family in identifying community and hospital resources and making referrals for follow up care as needed
  • Collaborates with the hospital clinical director and palliative team regarding hospital processes and protocol
  • Serves as a patient advocate

Requirements

  • Registered nurse currently licensed in Maryland
  • Current licensure as a Nurse Practitioner, Adult or Family Practice, in Maryland
  • Two (2) or more year’s hospice or home care experience preferred
  • Demonstrated knowledge of home care, hospice, palliative care, advance directives, pain management, symptom control, and discharge planning
  • CPR certified

Position:NURSE, PRN

Type:PRN

PRN shifts 8:00 a.m. – 8:00 p.m.

Responsibilities & Functions

  • Provides a consistent high quality hospice experience for every patient and family served
  • Augments Admissions Services staff by educating the patient/family regarding hospice services and completing the admissions process
  • Utilizes the nursing process and coordination with other IDT members to provide for the physical, spiritual, psychosocial, financial and bereavement needs of the patient and family
  • Communicates proactively with appropriate Team Leader/designee to ensure patient needs are met and smooth program operation
  • Coordinates with consultant pharmacist and attending physician to optimize symptom management
  • Assesses learning readiness and provides ongoing education and support to the patient/family according to their needs and the plan of care
  • Appropriately uses information technology to accurately and timely document in the patient electronic clinical record
  • Practices safe medication management according to professional standards which requires successful completion of medication mathematic skills competencies

Requirements

  • Current license to practice in Maryland as a Registered Nurse
  • Superior physical assessment skills
  • Current CPR certification for Healthcare Provider (BLS)
  • Ability to work independently, efficiently and to make sound nursing judgments utilizing critical thinking skills with a minimum of direct supervision

Position:MEDICAL SOCIAL WORKER

Type:Full Time

Seeking a Master’s prepared Medical Social Worker to join our compassionate interdisciplinary team providing a premier hospice experience for our patients and families. Manages a case load which requires travel to patients’ residences throughout our service area which includes Wicomico, Dorchester, Somerset and Worcester counties on Maryland’s lower Eastern Shore. Works collaboratively with other social workers to meet program needs. May assist in supervision/mentoring of social work interns, as needed.  LMSW, LCSW or LCSW-C required.

Position:HOSPICE AIDE - PRN NIGHTS

This is a PRN position with night shifts at our inpatient facility, Coastal Hospice at the Lake

Job Responsibilities & Functions
  • Provides direct patient care as instructed by a RN and/or physical therapist
  • Provides emotional support to the patient and family which requires effective human relation skills
  • Communicates effectively and timely with supervising nurse and other hospice aides by performing change of shift rounds
  • Assists with unit operations by:
    • Reporting low levels of linens and supplies
    • Maintaining safe physical environment according to unit needs and regulatory standards
    • Performing light housekeeping duties

Job Requirements

  • CNA or GNA. Requires at least one year previous experience in home care or geriatric setting.

Position:DIETITIAN

Type:PRN

Reports to the RN Team Leader.  Provides dietary consulting services to patients based on practice standards and Coastal Hospice policies.
