Supports the team by coordinating services to patients and families, facilitating communication between the team leader, team members, finance, patients, families, nurses and physicians.  LPN required due to responsibilities.  May be required to periodically visit patients or work a clinical rotation based on program needs.

General Statement of Duties:  Reporting to the Medical Director, is responsible for coordinating palliative care and consult service referrals at contracted hospitals; collaborates with hospice medical director and physicians, hospital and hospice management and staff, and palliative consult team members; receives clinical supervision from hospice Medical Director. The collaborating physician and Palliative Care Nurse Practitioner will complete palliative care consultations, evaluations and recommendations.

As a member of the Sr. Management Team, we will count on you for the leadership and direction of clinical services for Coastal Hospice.  You’ll ensure our patients and families receive the highest quality care compliant with all regulations and standards.  Direct oversight of four home hospice teams, an inpatient unit and a hospice residential facility.

Master’s Degree in nursing, healthcare administration or related field required along with a minimum of 2 years successful experience managing interdisciplinary clinical teams. Hospice and Palliative Care Certification preferred.

Type:Full Time

Full time, part time and per diem positions available.

Provides personal care services and administers medication to terminally ill patients at our hospice residential facility, The Macky & Pam Stansell House at Coastal Hospice at the Ocean.

3 – 12 hour shifts with weekend and holiday rotation required.  Must possess current Maryland CMT license AND CNA/GNA license.

Will consider CNA/GNA who are willing to attend CMT training and able to pass exam.

Seeking a Master’s prepared Medical Social Worker to join our compassionate interdisciplinary team providing a premier hospice experience for our patients and families. Manages a case load which requires travel to patients’ residences throughout our service area which includes Wicomico, Dorchester, Somerset and Worcester counties on Maryland’s lower Eastern Shore. Works collaboratively with other social workers to meet program needs. May assist in supervision/mentoring of social work interns, as needed.  LMSW, LCSW or LCSW-C required.

For our Home Hospice program; makes home visits to patients and assist with ADLs and light housekeeping.  Current Maryland CNA or GNA licensure, CPR, valid driver’s license and driving record that meets our standard required.
