Position:CERTIFIED NURSING ASSISTANT (CNA)

Type:Full Time or Part Time

Job Responsibilities & Functions:

  • Provides direct patient care as instructed by supervising nurse
  • Provides emotional support to the patient and family which requires effective human relation skills
  • Relieves the primary caregiver for occasional respite
  • Provides personal care, assistance with activities of daily living, homemaking and related assistance to patients
  • Communicates effectively and timely with supervising nurse and other hospice aides by:

– Documenting services clearly and accurately by the end of each shift
– Promptly reporting any significant change in patient’s condition

Requirements: 

  • Current CNA or GNA certification in Maryland
  • Current CPR certification for Healthcare Provider (BLS)

Position:Nurse Practitioner- Hospice and Palliative Care Team

Type:Full Time

Job Responsibilities & Functions:

  • Performs consults as needed in the inpatient and outpatient palliative care settings under arrangement with Coastal Hospice, Inc.
  • Assesses patient’s physical, emotional and spiritual needs and documents in the electronic medical record.
  • Prescribes appropriate diagnostic and/or therapeutic measures to achieve patient/family goals and documents in the electronic medical record.
  • Facilitates family meetings for goals of care and discussions of advanced directives.
  • Provides face-to-face visits to hospice patients to assist in determining hospice eligibility. This requires driving to individual patient homes throughout our 4-county service area.
  • Makes rounds on hospice general inpatient unit for the purpose of symptom management and education.
  • Promotes and provides education related to palliative care and end of life.
  • Participates in the development of data collection tools, communication processes, and other details of the consult service implementation and evaluation.
  • Collaborates across care settings to facilitate optimal coordination of patient-centered care.
  • Other relevant duties as assigned

Requirements: 

  • Current Maryland licensure as a Nurse Practitioner in adult or family practice.
  • Two years experience in related field preferred.
  • CPR Certification.
  • Proficiency in pain and symptom management.
  • Ability to safely and successfully perform the essential job functions consistent within the guidelines of ADA and other federal, state, and local laws.
  • Must be able to lift, push, pull, or move patients, equipment, and supplies in excess of 50# with or without the use of adaptive/assistive devices.
  • Knowledge and compliance with federal, state, local laws and guidelines related to this job description including OSHA, ADA, EEOC, and DOL, and other regulatory bodies.

Position:CERTIFIED MEDICATION TECHNICIAN

Type:Full Time or Part Time

Job Responsibilities & Functions:

  • Provides direct patient care and administers medication to the terminally ill patient and performs related tasks at Stansell House, per the plan of treatment as assigned by the case manager
  • Provides personal care, assistance with activities of daily living, homemaking/housekeeping, and related assistance to patients to increase the level of comfort, and to maintain personal hygiene and a safe, healthy environment for the patient and co-workers
  • Participates in weekend/holiday rotation to meet patient needs

Requirements: 

  • Possess a high school degree or equivalent and be eighteen years of age
  • Possess a current certification to practice as a Certified Nursing Assistant with the Maryland Board of Nursing (MBON)
  • Possess valid/current Certified Medication Technician certification with MBON
  • Current CPR certification for Healthcare Provider (BLS)

Position:REGISTERED NURSE

Job Responsibilities & Functions: 

  • Responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
  • Participates in weekend, holiday, on call and/or evening/night rotations
  • Educates patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed

Requirements: 

  • Licensed and currently registered to practice as a Registered Nurse in the State of Maryland or Delaware or other compact state
  • Current CPR BLS certification
  • BSN from an accredited college preferred
  • CHPN license expected within two (2) years of employment

Position:THRIFT SHOP ASSOCIATE

Type:Part Time

Essential Duties and Responsibilities:

Part-Time

Varied hours Monday through Saturday 10:00am-4:30pm

  • Under the direction and supervision of the Thrift Shop Operations Manager, provides support for the operation of the Thrift Shop
  • Prepares merchandise for sale and display; including sorting, washing, hanging, and moving to sales floor as appropriate
  • Prepares houseware items by hand cleaning or running through the dishwasher
  • Assists in removing seasonal items from the sales floor and follows Thrift Shop SOP for storing and recycling merchandise, seasonal items
  • Prices merchandise
  • Assists in ensuring volunteers are following sales floor processes
  • Displays merchandise in an appealing way
  • May open and close store as needed
  • May operate cash register as needed and compute end of day total cash register reports
  • Helps stock sales floor with inventory
  • Creates a positive image of Coastal Hospice in all customer transactions
  • Other duties as directed by the Thrift Shop Operations Manager or their designee

Education and Work Experience Requirements:

  • Customer service attitude; works well with others, relates positively and courteously with a variety of people and co-workers
  • Ability to maintain accurate records
  • Knowledge of grief process
  • Must be able to lift and carry up to fifty (50) pounds with or without the use of assistive devices
  • Ability to stand for up to four (4) hours

Position:HOME HOSPICE CARE COORDINATOR

Type:Full Time

Job Responsibilities & Functions:

Monday-Friday 8a-5p; On-call, Holiday, weekend, and off shift requirements

  • Provide a consistent high-quality hospice experience for every patient and family
  • Flexibility to manage the frequency and duration of visits based on patient/family needs
  • Coordinates with consultant pharmacist and attending physician to optimize symptom management.
  • Develops and amends the patient/family plan of care based upon identified problems and goals
  • Communicates timely and effectively with the Home Hospice Team Leader/designee, physicians, team members, patients, families, contracted vendors, and community.
  • Participates in the on-call rotation for night and weekend nursing coverage as assigned.
  • Practices safe medication management according to professional standards which requires successful completion of medication mathematic skills competencies.
  • Assesses learning readiness and provides ongoing education and support to the patient and family according to their needs and the plan of care.
  • Supervises and delegates to LPNs, hospice aides, students, and volunteers on the same assigned workday.
  • Rotates assignments as necessary to meet program needs.

Requirements: 

  • Current license to practice in Maryland as a Registered Nurse. BSN preferred.
  • Current CPR certification for Healthcare Provider (BLS)
  • Ability to work independently, efficiently, and to make sound nursing judgments utilizing critical thinking skills with a minimum of direct supervision
  • The ability to lift, push, pull or move patients, equipment, supplies, etc. in excess of 50 pounds with or without the use of adaptive/assistive devices.
