Home Hospice

Coastal Hospice provides home hospice care wherever you call home; whether it’s your home or the home of a family member, or an assisted living or nursing facility. Our hospice team members visit often, as determined by your unique needs.

How the Hospice Team Helps
  • Our doctors work with your own doctor or specialist to manage care and make house calls as needed
  • Registered nurses skillfully gauge how you’re doing, coordinate your care with your doctor and the hospice team and teach your loved ones how to care for you
  • Certified hospice aides help you with bathing, dressing, light housekeeping and other necessities
  • Social workers provide family counseling, advanced care planning and connections to other resources
  • Hospice chaplains provide non-denominational spiritual guidance and emotional support for the whole family
  • Trained volunteers offer friendship and comfort
  • Physical and occupational therapists help you maintain function and find ways to engage in life
  • Medical equipment, oxygen and supplies are delivered right to your door

How our Hospice Team Takes Care of Your Needs

Physicians at Coastal Hospice consult with your doctor to develop the best care plan for you.
If you need the hospice doctors, they will come to you.

Registered Nurses look after your care and medications and teach caregivers how to provide comfort.

Certified Nursing Assistants help you with bathing and dressing.

Medical Social Workers help you and your family find ways to cope with the effects of your illness, guide you in advance-care planning, and connect you with the resources you need.
On-Call Registered Nurses are available 24 Hours a day 365 days a year to come to your home.

Spiritual Counselors provide you and your family with guidance according to your faith.

Bereavement Counselors help you navigate the grieving process.

Physical and Occupational Therapists help you maintain strength and safety and find new ways to engage in daily life.

Volunteers who have the necessary education provide you and your family with friendship, comfort, and support.
