Six questions to ask yourself
- Have you or the person you’re concerned about visited the emergency room or hospital more than once in the past 6 months?
- Have you made more than one late-night or weekend call to the doctor?
- Are you too ill to keep doctors’ appointments?
- Have you stopped improving with medical treatments?
- Are you suffering from one or more complications? (For example, pneumonia, weight loss, or skin ulcers?)
- Do you need help to bathe, dress or eat?
