Guidelines for Hospice Eligibility
Coastal Hospice provides its services to eligible individuals throughout Wicomico, Worcester, Somerset, and Dorchester Counties, regardless of financial status or ability to pay. Patients must meet the following eligibility criteria for hospice care:
- Hospice is available for adults, children or infants.
- Patients whose life expectancy is approximately 6 months or less if the disease should run its usual course.
- Eligibility is reassessed on an ongoing basis and often hospice care can continue for beyond the initial 6 months.
- Prognosis is certified by a physician.
- Primary goal is the patient's physical and emotional comfort.
Some of the illnesses that meet medical requirements for hospice care include:
- Alzheimer's
- Amyotrophic Lateral Sclerosis
- Cancer
- Cardiovascular Disorder
- Dementia
- HIV/AIDS
- Liver Disease
- Stroke
Please note that this is not an exhaustive list. A member of the Access Team will meet with the patient and their loved ones to find the best way to meet their needs.
